Congratulations on winning a Beacon Award! We are looking forward to honoring you at an awards gala in October. Before that, however, we’d like to conduct a short interview with you for our Awards program and video presentation.

Our videographer, Chris Dobbins, will conduct your interview at a location of your choosing. We prefer locations like a place you volunteer at or your local Town Green, but Chris will accommodate most requests. Please indicate your preferred interview location using the form below.
